Comments:The mission highlighted the fact that the NTLP needs to prevent the production and transmission of MDR-TB, through continued strengthening of the general TB program to reduce risk factors for MDR-TB to an acceptable level. This can be achieved through the specific activities of DOT strengthening, regular drug supply provision and improved infection control. The National MDR-TB Guidelines are almost finished. SLD will be available once the GF Phase 2 contract is signed, and it is recommended that this is urgently addressed so as to rapidly implement the patient inclusion in treatment. Infection control remains problematic at Mulago hospital and must be corrected as soon as possible. A focal person for MDB-TB should ideally be maintained at central level, but the position is subject to availability of funds. On the positive side, staff has been trained in MDR-TB management and the waiting list has been updated.

The NTLP and partners have made enormous efforts to accomplish the outstanding recommendations made on former visits. Those that remain incomplete are due to the constraints of the available funds and scarce number of personnel. At District level the activities remain weak, but it is hoped that this will improve with the implementation of the round 6 Phase 2 GFATM grant.

Challenges remain, especially with regard to ensuring the essential funds in order to guarantee the accomplishment of the planned activities, the regular and timely supply of drug (FLD and SLD) and ensuring DOT in order to prevent further development and spread of MDR-TB.
