Background:
Initiation of the Practical Approach to Lung Health (PAL) implementation in Armenia has been started during the last two years, 2008-09. PAL national working group comprised of the representatives of the MOH, National Institute of Health (NIH), State Medical University, National TB Programme (NTP), and Chief Pulmonogist has been established to support the process. During this period, 3 missions by WHO EURO were organized to the country to support development of the PAL Guideline for Armenia, pilot training of the selected clinical preceptors on the use of the Guideline, and pilot testing of the Guideline in the field. As a result, the PAL Guideline for Armenia and the training manual for providers were developed, selected providers from central and peripheral levels were trained on how to use the PAL guideline and have pilot tested the PAL guideline, based on which the Guideline was revised. The available budget has allowed training of only 11 out of 60 clinical preceptors nationwide. In order to maintain the PAL momentum for 2010, it was decided to prioritize training of the remaining 49 preceptors, so that by the end of 2010 the entire “corps” of trainers in PHC has been trained in PAL, and additionally some TB and pulmonary specialists nationwide. Consider definite finalization of the PAL Guideline by the end of 2010 in line with the input and feedback by the 49 remaining preceptors (GPs). Overall, all stakeholders, including MOH representatives, members of the national working group, health providers, and international partners welcomed this work and found it very appropriate and relevant to the country needs. Along with better management of lung diseases, proper introduction and implementation of the PAL approach would allow early detection of and proper referral and management of TB in Armenia.

This year, through the funds provided under the PEPFAR/TBTEAM, it is possible to complete the training of trainers for 60 providers (including the remaining 49 clinical preceptors and some selected TB specialists), to further test the Guideline using the AGREE instrument, finalize it based on the feedback from the testing sites, and submit to the MOH approval for system-wide introduction and use.

Scope of work:
1. To act as an overall coordinator of the of the training of trainers (TOT) process on the use of the PAL (Practical Approach to Lung Health) Guideline in Armenia
2. To prepare and organize 9 training sessions for 60 providers selected from the primary health care facilities, who will act as master trainers on the use of the PAL Guideline in Armenia
3. To prepare for and conduct actual training sessions (6 sessions) for the selected 60 providers on the use of the Guideline
4. To prepare and conduct 3 briefing sessions on the pilot assessment of the PAL Guideline using the AGREE instrument
5. To compile feedback on the practical use of the Guideline received from the pilot sites from 60 providers using the AGREE instrument
6. To review feedback and address comments by modifying and finalizing the PAL Guideline
7. To finalize the PAL Guideline for Armenia coordinating the agreement on the final content of the Guideline with other experts.

Expected Outputs/Deliverables:
1. Training of trainers (TOT) agenda developed and TOT sessions for 60 providers conducted
2. PAL Guideline for Armenia is finalized and submitted to the WHO CO and the MOH.


Administrative issues:

1. Overall supervision of the work of the expert in the country is conducted by the Head of the WHO Country Office in Armenia (HCO WHO CO).
2. Electronic (MS World, Excel) copies of the documents (the Guideline, the training manual, agenda, comments from the pilot sites, etc) will be submitted by the expert in the Armenian or English to the WHO CO.
3. The expert shall not publish wholly or in part the findings or methods specific to this assignment at no circumstances.
4. Any deliverables/documents produced by the expert will be discussed and cleared with the MOH and WHO EURO and CO before final acceptance.
