Title:Clinical and programme audit

Terms of Reference:To recruit a consultant that would pay a one-week visit to the country with the following ToR:

- doing preliminary reading on TB control in SI, including data collected by NTP and other stakeholders on various aspects of the programme;
- meet on arrival with key stakeholders and discuss possible reasons of the abnormalities observed;
- travel to selected provinces and visit hospital, laboratories and peripheral clinics in order to gain full understanding of the way the programme works;
- provide practical, feasible advice to improve the situation at central and local level;
- suggest a list of operational research and pilot projects that would help identify the causes of the problems and fix them;
- de-brief NTP, MoH, WHO and other concerned partners before departure on the key findings of the mission;
- prepare and forward to NTP a detailed mission report within three weeks from return home.

Comments:Dr Nisbet assisted Solomon Islands to: (1) review the draft NTP guidelines and finalize them in a way to assure coherence and alignment with international standards on TB control; (2) develop appropriate formats for Recording & Reporting of PMDT and TB/HIV-related activities; (3) define technical specifications of medicines and consumables to be procured for the full implementation of the two programmes; (4) plan general Infection Control arrangements for MDR and TB/HIV; (5) assess the opportunity of piloting Community-Based TB Care in selected areas; (6) investigate the potential causes of a high number of initially 3+ smear-positive cases; (7) assess the cost-effectiveness of phasing in new diagnostic technologies; and (8) plan how best to target vulnerable groups, e.g. diabetes patients, with TB screening.

The NTP guidelines are a key document for the successful implementation of the Round 8 Global Fund grant as they set the standards and define the roles and responsibilities of all stakeholders in order to deliver quality TB care and services. They are closely related to the achievement of the Performance Framework targets, particularly those on case detection, cure and recording & reporting. The revised guidelines include chapters on items that have been included in Phase 2 of the grant, such as TB and diabetes, community TB care and infection control. Dr Nisbet also delivered a final version of the chapters on MDR-TB, TB/HIV and treatment of TB under special conditions for inclusion in the national guidelines. It is expected that the new guidelines will improve the way TB control is managed in Solomon Islands, particularly with regard to identification of new cases, clinical and laboratory follow-up of patients, recording and timely reporting of new cases and treatment outcome.
