| Comments: | Dr Satoshi Mitarai, Head of MD, PhD, Head, Bacteriology, Department of Mycobacterium Reference and Research, RIT (supranational laboratory) provided technical support from 15-20 May 2011 to (1) to assist National TB Reference Laboratory of Mongolia in introduction of liquid culture for diagnosis of TB, (2) to facilitate training on liquid culture for laboratory doctors and technicians at the national level. The consultant conducted an initial introductory training of mycobacteriual liquid culture and drug susceptibility testing (DST) with BACTEC MGIT 960 system, and a genotyping system with variable number of tandem repeat (VNTR) for national level laboratory staff of NCCD. The consultant also implemented laboratory practices using positive sputum specimens collected from patients and cultured M. tuberculosis strains. It was recommended that NTRL should perform liquid and solid culture examination for the clinical samples routinely for several months to obtain quality control indicators. The indicators are the proportion of culture-positive specimens among smear-positive specimens, and the contamination rate, at least. The consultant provided technica support in strengthening capacity of national level laboratory staff on how to conduct first-line anti-tuberculosis drug susceptibility testing (DST) using MGIT system. The consultant recommended to compare MGIT and conventional (proportion method) DST results using clinical specimens. In addition, it was strongly recommended to integrate MGIT culture and DST examinations into PMDT for the improvement of turn-around time for the diagnosis of MDR-TB as well as to conduct operational research for VNTR analysis which is a powerful tool for the genotyping of M. tuberculosis strains. |
