| Terms of Reference: | BACKGROUND Tuberculosis remains a major health problem in Kyrgyzstan. It is still necessary to improve the management of the TB programme both at the hospital and outpatient care levels, to improve treatment practices for effective TB control. For better implementation and organization of health care reforms in complicated economic and political situation, the Manas Taalimi National Health Care Reform Programme for years 2006-2010 was developed and adopted by the Government. The programme was extended for 2011. The main goal of the Manas Taalimi Programme was to improve the health status of the population through the creation of a responsive, efficient and integrated system of individual and public health services delivery. In the area of TB control the strategy emphasizes the need of further integration of TB control at primary health care (PHC) level, consolidation of TB control in the penitentiary system and expansion of detection and treatment of MDR TB. WHO comprehensive review of the National TB control in Kyrgyzstan was conducted in July- August 2010. The mission concentrated on the evaluation and follow up of the programmatic issues of TB control. Trends of TB epidemiological indicators, political commitment, funding and coordination and health system strengthening, TB case-finding and diagnosis, activities at PHC level, including DOTS strategy, drug management, integration of TB and HIV activities, TB monitoring and evaluation, PAL strategy, operational research as well as publications and educational activities were evaluated and reviewed. A technical WHO TB mission in July/August 2010 identified a number of actions and next steps for the overall improvement of the TB program. A strong advice of the WHO mission was to revise existing TB guideline and develop respective clinical protocols to ensure effective and qualitative TB services.
OBJECTIVE TA to revise TB guideline and develop clinical protocols to ensure effective and qualitative TB prevention, diagnosis, treatment and care.
SPECIAL TASKS The objectives of this contract are (i) to provide technical assistance in revision of national TB management guideline, (ii) to assist national TB specialists to develop clinical protocols: • Laboratory diagnosis • Pediatric Tb management protocol • In-patient management of TB patients • Out-patient management of TB, • Extra-pulmonary TB diagnosis and management
RESPONSIBILITIES OF THE CONTRACTOR AND EXPECTED OUTPUTS
1. To review respective normative documents. The Contractor will be responsible for revision respective clinical protocols and other documents related to TBI (programs, guidelines, protocols, reports etc). Materials to be reviewed must be in English and made available to consultant at least 15 days prior to the country visit.
2. To review quality of TB management, control and prevention. The Contractor will be responsible to visit respective facilities in Bishkek, the capital of Kyrgyzstan and region (based on national colleagues’ advice) meet and discuss with leading technical experts the current situation, challenges, opportunities regarding availability of and equal access to TB services.
3. To provide technical assistance in revision guideline and developing clinical protocols The Contractor will be responsible for revision guideline and management clinical protocols.
