Title:Country case study of pharmacy engagement to promote responsible use of TB drugs

Overall aim:
The overall aim of the case study is to provide an example of how prescription and dispensing of TB drugs can be restricted to selected health care facilities and professionals with a focus on pharmacies. The case study should provide lessons for national TB programmes that are facing challenges with uncontrolled, irrational, and harmful use of TB drugs, which may contribute to poor treatment results, drug-resistance development and amplification, adverse drug reactions, and high costs to patients.

The specific objectives are:
1.To describe the current situation of TB drug availability, especially with regards to:
•availability of TB drugs outside the national TB programme especially private pharmacies and care providers
•TB drug prescription rights by various categories of prescribers
•Procurement and distribution practices
•Dispensing practices
•Legal framework and specific regulations for restricting availability of TB drugs, (as well as similar regulations for other antibiotics, and other categories of medicines)

2.To assess changes in prescribing and dispensing practices associated with the introduction of restriction of TB drug availability in private pharmacies.

3.To analyze the process of restricting TB drug availability to NTP or NTP-affiliated facilities only, with special attention to:
•Critical conditions and events that enabled the successful implementation of the policy
•Barriers and how they were overcome
•Any remaining problems, shortcomings, resistance, and barriers, especially with regards to 2nd line drugs
•Lessons learned for other countries

Approach
A consultant will conduct a field visit during five working days. In preparation for the field visit, the consultant will liaise with respective NTP, the WHO country office, WHO regional office, and the Stop TB Department at WHO HQ in order to plan the detailed itinerary, plan meetings with key informants, and identify key background documents that could be reviewed before the travel. A full report will be completed within four weeks from the completion of the field work. WHO HQ, Stop TB Department (TBS) will finance the study through an APW with the consultant.
