Title:Situational analysis on community based TB activities supported by the GF
Type:Mission
Terms of Reference:The consultant assisted the Community Engagement team of the Stop TB Department of WHO in an effort to facilitate and catalyse smooth implementation of community based TB activities under AMREF as the Principal Recipient, and to ensure the outcomes are reported to the NTP through a standardized set of indicators.
The specific objective of this consultancy was to conduct country-specific situational analysis to identify barriers and solutions to catalyse the implementation of GF supported community based TB activities, examining the following areas:
i. community-based strategy (including description of community based model, definition of roles and responsibilities of different stakeholders),
ii. implementation plan (including collaboration with NTP in planning, coordination, supervision, training and M&E),
iii. budget, financial reporting, grant performance and all its components,
iv. M&E for community TB care (including data collection and flow, data quality and contribution to treatment adherence and case detection),
v. strengths, weaknesses, opportunities and threats.

Comments:Consultancy outcomes include the following findings and recommendations, which will be integrated in the BMS-supported initiative in Kenya to scale-up the engagement of NGOs is community based and integrated TB/HIV activities:
• There is need to have a standardized and fully operationalized CTBC strategy and engagement of CSOs framework by:
o Setting up of a national and decentralized mechanism for CSOs engagement to design and effectively implement CTBC projects
o Developing a national essential package of CTBC services to encourage standardization and improve quality of services provided
o Strengthening and operationalizing CTBC related national M&E system to ensure effective data acquisition and analysis to inform planning and implementation of activities.
o Generating annual reports showing linkages of CTBC activities’ contribution to patient care and overall TB control to ensure due recognition is given to those contributing to the national TB control efforts.

• There is need to strengthen the organizational capacity of CSOs involved and/or wishing to get involved in CTBC activities to more effectively design and implement projects in the community to contribute optimally to the national indicators.

• There is need develop and implement an advocacy campaign targeted at the health providers serving at the health facilities to accept the important role played by the Community Health Workers (CHWs), and especially in referring of TB suspects and collection of sputum samples, to ensure successful implementation of the CTBC strategy.

• There is need for DLTLD to develop innovative and more sustainable financing approaches for CTBC activities in the country and reduce the current over-dependence on donor funds

• There is need to strengthen linkages of CSOs-run CTBC activities to existing programs for more synergy and effective service provision.
