| Terms of Reference: | Goal Provision by independent international expert to the Main Medical Department of the Ministry of Justice (MMD MoJ) the first hand experience in DR-TB management, infection control and organization of TB control in prison settings, by documenting and analysis in details penitentiary system’s first cohort of 200 DR-TB patients treatment outcomes and provide progressive experience and recommendations for further improvement.
Objectives • To comprehensively analyze first cohort data of 200 DR-TB patients; • To analyze reasons of drug resistance amplification during the treatment course; • To prepare an analyses report with critical judgment on outcomes and comparison with available data from literatures; • To prepare recommendations on ongoing DR-TB treatment and information flow management for DOTS-plus program in penitentiary system; • To prepare grounds for publishing an article in international medical issues in agreement with MMD MOJ.
Desired output from analysis • Description of DR-TB cases enrolled in Cat IV treatment: DST profiles, previous treatments, spread of the process and severity of the disease; • DR-TB treatment regimens etc; • Analyze in details DR-TB treatment outcome results of the first 200 DR-TB patients cohort; • Description and rationale of integration criteria for enrollment to Cat IV treatment over time; • Delays between DR-TB diagnosis and starting Cat IV treatment: mean duration of delays, reasons etc.; • Analyze reasons for successful/unsuccessful and adverse treatment outcome (side effects and its management, incentives and enablers etc); • Document number of diagnosed DR-TB cases refusing Cat IV treatment and recommendations for further steps to determine underlying causes; • Document separately treatment outcome of released detainees continuing Cat IV treatment in civilian sector; • Judge on possible DR-TB strains resources spreading and ways of its identification
Requirements: • Data bases analysis and verification experience; • Working experience in the region and with similar risk groups; • Computer literacy and well-version in EPINFO 6 statistical software • Strong knowledge in WHO MDR-TB management requirements; • Previous experience in similar missions; • Publications in internationally accepted issues is an asset; • Fluency in English/Russian.
Time schedule The Consultancy should take place from end May to mid June 2012; final report should be submitted to MMD MoJ by the end of June.
