Comments:The National Tuberculosis and leprosy Control Programme (NTBLCP) conducted a mid-term review between 8-18 April 2013 of its national strategic plan. The author attended the briefing and de-briefing meetings in Abuja on the 1st and last days; 9-14 April were spent in Lagos, and 15-17 April in Ibadan.
The reviewers were divided into 6 teams to visit 34 LGAs in 9 states. In each state, the State TBLCP Coordinators were interviewed and health facilities assessed using observation and interview checklists. The author’s team also visited central and state medical stores.
Overall review findings: TB patients are well-followed and treated once they enter the system with a 85% treatment success rate. However, not enough AFB microscopy services are available to reach all patients (currently only serving 1 : 122,000 instead of 1 : 80,000 population). The quick roll out of Xpert MTB-RIF is diagnosing more MDR TB patients than can be accommodated for treatment under the current policy of 8 months hospitalization. Plans are underway to strengthen logistics and psychosocial support mechanisms to enable earlier release from the hospital. Childhood TB has yet to be fully addressed with very few doctors having been trained and paediatric drug formulations in stock were expiring in April 2013, with the next order held at port; however, the latest worker’s manual includes algorithms for diagnosis and treatment of paediatric TB. Supervision at all levels needs to be strengthened to improve quality of diagnosis and treatment of patients, and recording and reporting, through on-the-job training and feedback of health facility staff.
