| Terms of Reference: | Globally, data are lacking on the true burden of childhood TB because children are more likely to have paucibacillary, disseminated or extrapulmonary forms of disease, and to be sputum smear-negative; therefore, TB in a child is often very difficult to diagnosis definitively, and is commonly under-diagnosed and not captured in surveillance estimates that focus on sputum smear-positive disease. TB surveillance systems the world over are attempting to improve the diagnosis and reporting of pediatric TB.
Botswana has among the highest rates of TB in the world with an estimated TB incidence of 503 TB cases per 100,000 population in 2010. Approximately 65% of TB patients in Botswana have HIV. The current burden of childhood TB and TB/HIV is not precisely known. Improving childhood TB diagnosis and management has been identified as a priority by the BNTP. A national pediatric TB diagnostic algorithm was recently developed and revised guidelines for the management of TB were released in late 2011, and include a section on pediatric TB. The effect of these guidelines has not been evaluated. This planned evaluation aims to better characterize the burden of TB and TB/HIV among children in Botswana and provide baseline data for future evaluation of efforts to improve the diagnosis and management of childhood TB; to assess levels of contact tracing of children (both children as TB contacts and as TB index cases) and to identify barriers to the provision of TB and TB/HIV services for children. A convenience sample of TB diagnosis and treatment facilities will be selected and visited. Data will be collected from these selected TB facilities to document burden of TB disease in children, document contact tracing and for comparison to national level data to characterize the fidelity and timing of data transfer. Data from the national level database (the electronic TB registry) will be reviewed to determine usefulness of the data and associations with outcome.
