Title:association between diabetes and tuberculosis in Kiribati.

Terms of Reference:The incidence of TB in Kiribati is the highest in the World Health Organization (WHO) Western Pacific Region, and has remained persistently elevated since the early 1990s. Studies have shown a reproducible association between diabetes and risk for active TB disease, and the high prevalence of diabetes in Kiribati may contribute substantially to the unresolved TB epidemic there.

In August 2010, we initiated a study to document the association between diabetes and tuberculosis in Kiribati. The study was commissioned by the Secretariat of the Pacific Community (SPC), and CDC was brought on as formal technical advisors and study co-investigators. The study design is case-control; control patients were randomly selected from the community and cases were TB patients registered for treatment over a defined time period. In addition to the case-control study, we planned to conduct a cohort analysis, comparing presentation and outcomes among TB patients with and without diabetes.

Staff from the National TB Program (NTP) were selected and hired to participate in this study and training was conducted in August 2010. Controls were enrolled between August and October 2010. TB patients were considered eligible cases if they were over 17, from the capital island and registered for care between June 2010 and February 2011. Enrollment of cases was started in October 2010 and continued through February 2012.
