Title:To plan for upcoming study of TB diagnostics in children in Nyanza Province, Kenya.

Terms of Reference:h burden settings, TB is an important cause of disease and death among children and a preventable cause of under-5 mortality; HIV infection is an important risk factor for TB disease and is associated with an increased risk of death. However, it is difficult to confirm the diagnosis of TB in children. Diagnosis of TB is particularly difficult among children with HIV infection. Mycobacterial culture, the gold-standard for laboratory confirmation of TB disease, has low yield in children, often 10% or less.

We are currently preparing for a study in Nyanza District, Kenya, in collaboration with KEMRI/CDC to determine the optimal approach to laboratory diagnosis of TB disease in children by systematically applying currently available diagnostic tests (including sputum smear, culture, Xpert MTB/RIF assay, and other available PCR-based techniques) to a panel of specimens in the evaluation of children both with and without HIV infection. In addition, we will simultaneously assess test characteristics of emerging diagnostic tools for TB. Understanding the optimal approach to laboratory diagnosis of TB in children will allow establishment of an improved approach to diagnosis against which new approaches to diagnosis and screening can be compared.

The primary goal for this TDY was to prepare for this upcoming study. Key issues to address were: preparation of standard operating procedures for specimen collection and testing, study staffing, training of study personnel, procurement of supplies for specimen collection and testing, planning study flow, planning study data management.
