| Terms of Reference: | The epidemiology of pediatric drug-resistant TB is not well described outside of the Western Cape province of South Africa, thus our collaborative group (The South African Medical Research Council, National Health Laboratory Service and the National Department of Health) has conducted a study to describe drug-resistant TB in children in four provinces in the eastern part of the country to generate baseline data in high-burden provinces in the rest of the country. One component of this study was to evaluate the existing surveillance system for drug-resistant TB, determining whether a child treated in a drug-resistant TB hospital is listed in hospital admissions log, facility drug-resistant TB registers, national electronic drug-resistant registers, and the national reference laboratory database. Further, as part of the study, CDC-ATL and MRC-South Africa have begun discussions with key stakeholders on the importance of sharing current research, innovative program practices, and lessons learned regarding PDR-TB in South Africa. The current plan is to hold this dissemination meeting in November 2012, in partnership with the NDOH. 2. Mozambique has one of the highest documented rates of multidrug-resistant TB (MDR TB) in Africa. A recent rapid drug resistance survey indicated the prevalence of MDRTB has remained stable since 1998 at 3.5% among new cases, 11.2% among re-treatment cases. According to WHO, in Mozambique, an estimated 1700 (1100-2800) patients had MDR TB among the notified pulmonary TB cases in 2009. Of these, only 140 MDR TB cases were confirmed and 103 of those patients initiated treatment. CDC-ATL and CDC-Mozambique have been working with the national TB program (PNCT) to conduct a program evaluation of MDR TB management to characterize diagnostic methods, time between diagnosis and treatment, treatment practices, and treatment outcome among patients treated for MDR TB between 2007-2011. Outcomes and Accomplishments South Africa: ? Assisted in successful oral presentation by study partners at the South Africa TB Conference. ? Met with study partners to plan next steps for dissemination of study results for the PDR-TB Study. ? Met with key stakeholders in South Africa to plan next steps for a meeting of academics, policymakers, and program managers to review current state of research regarding PDR-TB in South Africa and how to turn research into practice. Mozambique: ? Finalized data collection tools and training manuals for data collection training for the MDR TB management evaluation. ? Trained a team of 15 data collectors and team supervisors on basics of data collection, purpose of the evaluation, and specific tools for the assessment. ? Piloted data collection at two facilities treating MDR TB. ? Established a plan for CDC/GAP Mozambique to troubleshoot data collection issues to promote consistency among the different data collection teams. |
