Title:Programme Review of NTP - Nepal CONSULTANT FOR THE TB/HIV COMPONENT

Terms of Reference:Background

The prevalence of HIV/AIDS in Nepal is still low, between 0.3% and 0.5% in the general population. Results of a sentinel site survey conducted in 2006 pose the estimates of TB patients with HIV infection at 2.4%. Since PITC is only offered to a small proportion of TB patients identified as “HIV suspects”, figures from routine testing do not yield a reliable picture of the prevalence of TB/HIV in Nepal.

In spite of an increasing collaboration between the TB and the AIDS programme and the existence of recently updated TB/HIV policy, guidelines and SOPs, there are many bottlenecks towards the adoption of the WHO recommendations for the effective implementation of TB and HIV collaborative activities. It seems, in particular, that health workers are reluctant to offer HIV testing to all TB patients and that PLWHA are only erratically screened for active TB and if found TB-free are not offered IPT for six months. It is also commonly seen that co-infected patients are not started on ART when diagnosed with TB although this is spelled out very clearly in the national policy.
The further expansion of Xpert technology, planned for early 2013, will allow earlier and easier detection of active TB and MDR/TB in PLWHA but will also create more demand for collaboration between the two programmes.

Against this background, the consultant, working in close collaboration with NTC and the concerned partners will:

• Review the information and data available on TB and HIV in Nepal from literature and locally available reports;

• Review the current status of TB/HIV collaborative activities with the manager and the TB/HIV focal point of both programmes;

• Question regional and district TB and HIV staff about the current arrangements for HIV testing & counseling among TB patients, TB screening for PLWHA and clinical management of co-infected patients;

• Identify legislative, cultural, technical and logistic obstacle to the full implementation of WHO TB and HIV collaborative activities;
• Prepare concrete, feasible recommendations and a road map towards the implementation of TB and HIV collaborative activities in Nepal.


MINIMUM REQUIRED QUALIFICATIONS OF THE CONSULTANT

Educational qualifications

- University degree in health-related science
- Master in Public Health or equivalent title / working experience
- Training on TB control

Work experience

- At least 10 years of employment in organizations engaged in public health and disease control, including HIV and TB.
- At least 7 years of international experience in low-income countries
- Previous experience as consultants for NTP or other programme reviews

Skills

- Excellent knowledge of spoken and written English and capacity to produce high level formal documents and effective presentations in this language
- Capacity to produce results working in a team, under pressure and within a limited time frame.
