Title:Programme Review of NTP - Nepal CONSULTANT FOR THE INFECTION CONTROL COMPONENT

Terms of Reference:Background

The association of TB and HIV/AIDS, the lack of concern paid to TB transmission in health-care and congregate settings, and the absence of a global TB infection control strategy have created a suitable environment for transmission and spread of MDR-TB and extensively drug resistant TB (XDR-TB), as well as drug-susceptible TB among patients, health-care workers and the community.
TB infection control is a combination of measures aimed at minimizing the risk of TB transmission within populations. The foundation of such infection control is early and rapid diagnosis, and proper management of TB patients. TB infection control requires and complements the implementation of core interventions in TB control, HIV control and strengthening of health systems. In addition, countries should include TB infection control in their national infection prevention and control policies, and should maximize synergies between programmes that deal with infection prevention and control, and those focusing on TB and HIV control. TB infection control cuts across disciplines.
The specific TB infection control recommendations are described in the document WHO policy on TB infection control in health-care facilities, congregate settings and households(WHO, 2009). They include four major activities:
• managerial activities at the national and health facility levels;
• administrative control;
• environmental controls;
• personal protective equipment.
Infection control measures at the national and local or facility level are also described in the framework document for implementing the WHO TB IC policy (WHO, 2010). This document is a practical tool for implementation and monitoring of TB IC measures.

Objective

Against the above background, the reviewer should comment on:

• the administrative measures in place, especially for enabling early detection of active TB cases;
• measures in place at health facility level for reducing the transmission of Mycobacterium by infectious cases;
• existence of measures implemented in relation to health-care workers.
• next practical steps to implement the national guidelines on Infection Control.
• most suitable indicators to measure outcome and impact of IC activities.
MINIMUM REQUIRED QUALIFICATIONS OF THE CONSULTANT

Educational qualifications

- University degree in health-related science or engineering/architecture with post-graduate training on infection control;
- Master in Public Health or equivalent title / working experience
- Training on TB control and/or strong understanding of TB control strategies

Work experience

- At least 5 years of experience in infection control applied to disease control, particularly TB and other airborne diseases and HIV;
- At least 5 years of international experience in low-income countries
- Previous experience as consultants for NTP or other programme reviews

Skills

- Excellent knowledge of spoken and written English and capacity to produce high level formal documents and effective presentations in this language
- Capacity to produce results working in a team, under pressure and within a limited time frame.
