Organization Description:
The Community Health and Development Initiatives is a community based organization established and registerd with local and state Government, with clear focus on development in the social sectors.We really engaged stakeholders on identified issues that has to do with Development and advocate evidently so as to get the desired change in our community.
2.Our attention was tilted towards Tuberculosis as a result of mapping conducted by an NGO working on TB and we then understand the gravity of the disease and the need to sensitize our people on the dangers of it.
3.As at the time of filling this template our Organization(COHDI)is involved in the program of Active Case Findings (ACF)a Global fund interventions in slums(Nomadic Settlements) areas in Gwaram Local Government area of Jigawa state.

TB Care Delivery:
By supporting patient to access services at the nearest Health Facility by the means of engaging Community TB workers.

Drug-Resistant TB:
in following it up with MDR-TB patient and placing him a Tretment supporter

TB-HIV:
On the course of our search for TB patients all those identified with HIV were referred to appropriate HIV/AIDS counseling and treatment units for further action

Laboratory Strengthening:
We do also support TB patients in trying to comply with TB sputum collections as prescribed by Lab officials
