Organization Description:
Sihanouk Hospital Center of HOPE provides a center for the further education and clinical training of medical professionals, while delivering 24-hour high-quality, free medical care for the poor and disadvantaged in Cambodia.

The TB Clinic at the Sihanouk Hospital developed out of a growing need to treat our HIV/AIDS patients for TB. To improve adherence and cure rates, SHCH adopted the Direct Observe Treatment Services (DOTS) strategy and became an official National Tuberculosis Program (NTP) site beginning in August 2001. SHCH has innovated a number of Active Case Finding methods with high risk impoverished populations and also has state of the art diagnostic capabilities.

Please tell us how your organization is contributing to your country's national TB control plan:
SHCH’s TB Clinic, which opened in 1999 and became an official National TB Program (NTP) site in August 2001. SHCH’s activities include daily home-to-home and community TB and TB/HIV education; identification and referral of TB suspects; TB diagnostic services; home and facility based DOTS; adherence counseling; and defaulter tracing. SHCH expanded our successful community-based TB program to the North Operational district (NOD) of Phnom Penh. Working in close cooperation with NOD TB program supervisors, local health centers, and the NOD referral hospital, SHCH’s TB team provides support in the areas of TB screening, patient tracing, DOTs, and TB recording and reporting. The team also provides food and transportation support for severely ill and/or poor TB patients and daily community based educational sessions for more than 5,800 community members annually on topics such as TB, TB/HIV, tobacco use, and other airborne illnesses.

SHCH also provides technical support to all TB unit and laboratory staff in the NOD, helping to improve their skills in smear-making, providing refresher training, and linking TB suspects with CXR, ultrasound, or other investigatory procedures for extra-pulmonary TB diagnosis. SHCH’s comprehensive efforts have proven effective in increasing TB screening and diagnosis in the NOD.

With support from the WHO’s Stop TB Partnership, SHCH launched a 1 year project spanning all four Operational Districts of Phnom Penh geared at increasing case notifications in poor urban settlements of Phnom Penh through a strategy that deploys trained village health volunteers to conduct intensified , house-to-house case finding; reduces delays in the diagnosis and treatment of TB by expediting diagnostic logistical chains; uses mobile technologies to enable rapid case notification from the top to the bottom of the health services pyramid; and uses new GeneXpert technology to expedite diagnosis of smear negative and MDR TB.

TB Care Delivery:
SHCH’s TB Clinic opened in 1999 and became an official National TB Program (NTP) site in August 2001. SHCH’s activities include daily home-to-home and community TB and TB/HIV education; identification and referral of TB suspects; TB diagnostic services; home and facility based DOTS; adherence counseling; and defaulter tracing.

TB-HIV:
SHCH’s medical staff have conducted many clinical and operational studies related to TB/HIV and MDR-TB and have served as the principal investigator for a WHO study on the implementation of an algorithm— as proposed in the 2006 WHO guidelines— to improve the diagnosis of TB among adults in HIV prevalent, low resource settings, in collaboration with CENAT and ITM.

Laboratory Strengthening:
SHCH has a long-standing partnership with the Institute of Tropical Medicine in Antwerp, which has resulted in significant capacity building in both human resources and infrastructure in HIV, MDR TB, and microbiology.
